Active Travel Social Prescribing
Our FREE programme is designed to help adults to begin to make cycling and walking part of their everyday lives. It will support you to use free tools and links provided by our team to make active travel decisions and achieve sustainable travel goals.
You get the 1-1 support of a Health Improvement Practitioner. They will work with you to discuss how you can build walking, cycling and wheeling into your everyday lives. And maybe develop a personalised travel plan with you. They will identify existing walking and cycling provision that you can access and support you to access it. The Health Improvement Practitioner will work with you to help identify your barriers to active travel.
Referral is essential - Just ask your GP or Social Prescriber to refer you using the Professional Referral Form on the Healthy Cornwall website.
Referral Criteria:
You need to be a registered patient with GPs in Penzance, St Austell & The China Clay Area or Bodmin.
You also need to be one or more of the following:
- An adult seeking to improve their mental health and wellbeing,
- An adult with poor physical health (including long term health conditions)
- disabled people (adults)
- an unemployed adult
- Aged 50+
