Active Travel Social Prescribing
Our FREE programme is designed to help adults to begin to make cycling and walking part of their everyday lives. The team supports them to use the free tools and links provided to make active travel decisions and achieve sustainable travel goals.
Clients get the 1-1 support of a Health Improvement Practitioner; they will work with them to discuss how they can build walking, cycling and wheeling into their everyday lives. Additionally, they may develop a personalised travel plan for the individual. They will identify existing walking and cycling provision for the individual to access and support them to access it. The Health Improvement Practitioner will also work to help identify barriers to active travel and how they can be overcome.
Referral is essential. Please see referral criteria below.
Referral Criteria:
Individuals must be a registered patient with GPs in Penzance, St Austell & The China Clay Area or Bodmin.
Referred individuals must also be one or more of the following:
- An adult seeking to improve their mental health and wellbeing
- An adult with poor physical health (including long term health conditions)
- Disabled people (adults)
- An unemployed adult
- Aged 50+
